So if you are planning to play a sport at any time during the 2024-2025 school year, call and get an appointment ASAP. Sports physical appointments book up very quickly so don’t delay and call today. All students MUST have a consent on file with us AND have a sports physical form completed by parent or guardian. If they come to an appt without these, we will not be able to complete the sports physical and it will be rescheduled. See below for links to forms. You can also pick them up from the school health center as they are located outside of the health center door.
